Default "title" attribute of form controls to "la_fld_{FieldName}"

      Description

      I've recently discovered, that we have some minor blocks in form_blocks.tpl template:

      1. when block "inp_edit_date" is used omitting "title" attribute, then automatic title attribute has value "la_fld_FieldName_date" instead of "la_fld_FieldName"
      2. no way to pass "empty_label" parameter (that was added recently) to "inp_edit_options" block
